Pelayanan kefarmasian di apotek naungan PT SKA melibatkan berbagai aktivitas operasional yang dilakukan secara rutin, seperti pengelolaan stok obat, proses pemesanan dan penerimaan barang, serta pelayanan resep dan nonresep kepada konsumen. Dalam pelaksanaannya, masih terdapat beberapa proses kerja yang bergantung pada ketelitian tenaga kefarmasian dan belum seluruhnya didukung oleh prosedur operasional yang baku, sehingga berpotensi menimbulkan risiko dalam operasional apotek. Penelitian ini bertujuan untuk mengidentifikasi risk event dan risk agent pada pekerjaan kefarmasian, menentukan risk agent yang menjadi prioritas, serta menyusun usulan aksi mitigasi yang sesuai dengan kondisi apotek naungan PT SKA. Metode yang digunakan adalah metode House of Risk (HOR) dengan data dikumpulkan melalui wawancara, observasi lapangan, dan pengisian kuesioner oleh apoteker serta supervisor penanggung jawab. Hasil penelitian menunjukkan bahwa terdapat 26 risk event dan 20 risk agent, dengan 10 risk agent ditetapkan sebagai prioritas, serta dihasilkan 14 usulan aksi mitigasi yang difokuskan pada penyusunan Standar Operasional Prosedur (SOP), peningkatan kepatuhan tenaga kefarmasian melalui penekanan aturan kerja, serta perbaikan pengendalian operasional seperti penjadwalan rutin dan penertiban administrasi, sehingga risiko serupa tidak terus berulang. abstract: Pharmacies are pharmaceutical service facilities that play an important role in medicine management and the provision of health services to the community. Pharmaceutical services at pharmacies under PT SKA involve various routine operational activities, including drug inventory management, ordering and receiving products, as well as providing prescription and non-prescription services to consumers. In practice, several work processes still rely heavily on the accuracy of pharmacy personnel and are not yet fully supported by standardized operating procedures, which may lead to operational risks within the pharmacy. This study aims to identify risk events and risk agents in pharmaceutical activities, determine priority risk agents, and propose appropriate risk mitigation actions in accordance with the operational conditions of pharmacies under PT SKA. The House of Risk (HOR) method was applied in this study, with data collected through interviews, field observations, and questionnaire distribution to responsible pharmacists and supervisors. The results show that 26 risk events and 20 risk agents were identified, with 10 risk agents classified as priorities.
