Pharmaceutical services at pharmacies under PT SKA involve various routine operational activities, including drug inventory management, ordering and receiving products, as well as providing prescription and non-prescription services to consumers. In practice, several work processes still rely heavily on the accuracy of pharmacy personnel and are not yet fully supported by standardized operating procedures, which may lead to operational risks within the pharmacy. This study aims to identify risk events and risk agents in pharmaceutical activities, determine priority risk agents, and propose appropriate risk mitigation actions in accordance with the operational conditions of pharmacies under PT SKA. The House of Risk (HOR) method was applied in this study, with data collected through interviews, field observations, and questionnaire distribution to responsible pharmacists and supervisors. The results show that 26 risk events and 20 risk agents were identified, with 10 risk agents classified as priorities. Furthermore, 14 risk mitigation actions were proposed, focusing on the development of Standard Operating Procedures (SOP), strengthening compliance among pharmacy personnel through enforcement of work rules, and improving operational control such as routine scheduling and administrative standardization, in order to prevent the recurrence of similar risks.HD Industries.
