To fulfill requirements for obtaining MD, PT XYZ is required to Good Manufacturing Practices (GMP). This study aimed to identify and evaluate implementation of GMP at PT XYZ based on BPOM guidelines. This research employed descriptive and experimental methods, with raw and fried fishballs as the samples. This evaluation covered 24 aspects and 66 clauses of the food production facility assessment. The results showed that GMP compliance rate was 81.82%, resulting in a total non-conformity score of 17 (7 minor and 5 major). These findings indicated that the implementation of GMP at PT XYZ fell into Category B (Good) and met the requirements for obtaining a CPPOB license. Based on root cause analysis, several improvement recommendations were proposed, which included repainting the walls; repairing cracked floors; installing a new ceiling; rearranging machines and equipment according to the production flow; separating the raw material handling room and the packaging room from the processing area; providing complete pest control facilities; providing routine employee training delivered by competent external trainers; establishing a routine training schedule; constructing an enclosed packaging room; providing training related to cross-contamination; providing cold storage for raw material storage; repairing the cold storage floor; and conducting socialization and inspections regarding the implementation of building maintenance SOPs, room sanitation SOPs, pest control SOPs, personnel training SOPs.
