description: Background: End Stage Renal Disease (ESRD) patients undergoing routine  hemodialysis often experience anxiety due to dependence on dialysis machines, lifestyle  changes, and future uncertainty. Untreated anxiety can affect treatment adherence and  patients' quality of life. Objective: To determine the effect of Guided imagery on anxiety  levels of ESRD patients undergoing routine hemodialysis at Dr. Adjidarmo  Rangkasbitung Hospital in 2025. Methods: This study used a pre-experimental one  group pre-test post-test design with purposive sampling technique on 34 respondents.  Anxiety levels were measured using the Hamilton Anxiety Rating Scale (HARS)  questionnaire before and after Guided imagery intervention. Data were analyzed  Paired Sample t-Test. Results: Before intervention, the majority of respondents  experienced moderate anxiety (67.6%) and mild anxiety (32.4%). After intervention,  there was a decrease to mild anxiety (50.0%), no anxiety (47.1%), and moderate anxiety  (2.9%). Statistical test showed p-value = 0.000 (p < 0.05) Conclusion: There was a  significant effect of Guided imagery on reducing anxiety levels of ESRD patients  undergoing routine hemodialysis. Guided imagery can be recommended as a  complementary nursing intervention for anxiety management in ESRD patients.
