description: Background: Patients with End-Stage Renal Disease (ESRD) undergoing hemodialysis often experience excessive thirst due to fluid intake restrictions, which can reduce patient comfort and adherence. Objective: This study aimed to determine the effect of ice cube therapy on the level of thirst in patients with ESRD undergoing hemodialysis at RSUD Adjidarmo. Methods: This study employed a pre-experimental design with a one-group pretest–posttest approach. A total of 34 respondents were selected using purposive sampling. Thirst levels were measured using the Thirst Distress Scale (TDS) before and after the administration of ice cube therapy. Data were analyzed using a paired t-test with a significance level of α = 0.05. Results: The findings showed a statistically significant reduction in thirst levels after the administration of ice cube therapy, with a p-value < 0.05. Conclusion: Ice cube therapy is effective in reducing thirst among patients with ESRD undergoing hemodialysis. Recommendation: Ice cube therapy is recommended as a routine non-pharmacological nursing intervention, and further studies are suggested to use experimental designs with control groups to strengthen the scientific evidence.
