description: Background: Pregnancy is a natural process that can cause risks, especially in high-risk pregnancies. High-risk pregnancies require optimal physical and psychological readiness so that the delivery process runs safely. Objective: to determine the relationship between the level of knowledge of pregnant women about high-risk pregnancies and childbirth readiness in the Serang City Health Center area in 2025. Method: The study used a quantitative approach with a cross-sectional design. A total of 55 pregnant women respondents were included by filling out a questionnaire with a Likert scale. Results: The study showed that the majority of respondents were 24-28 years old, had a high school education, and worked as housewives. Most respondents had a low level of knowledge regarding high-risk pregnancies, and showed low childbirth readiness. The chi-square statistical test produced a p value = 0.000 which showed a significant relationship between the level of knowledge and childbirth readiness. Conclusion: The low level of knowledge of pregnant women has a direct impact on the lack of readiness in facing childbirth. It is recommended that health workers increase their educational role for pregnant women in order to improve their knowledge and better childbirth readiness, especially in anticipating the risks of pregnancy that can occur.
