Low understanding of concepts will have an impact on students' success in understanding the material. To determine the level of students' conceptual understanding, a test instrument is needed. Instruments that are able to measure students' complete understanding of concepts, such as implementing the 4TMC- based diagnostic test instrument assessment. The aim of this research is to determine the level of validity and teacher response to the Zoho Forms- based 4TMC diagnostic test instrument to measure the level of understanding of junior high school students' concepts on the moving theme we live. This development research refers to model modified from Sugiyono which consists of 6 stages, namely: 1) potential problems, 2) data collection, 3) product design, 4) design validation, 5) design revision, 6) product testing. The expert subjects and samples in this research were 3 lecturers as evaluation experts, and 3 science practitioners, as well as 44 junior high school students VIII Data was collected using interview sheets, validation sheets, and teacher response questionnaires to determine the validator's assessment of the test instruments developed. Based on the expert validation results of the 4TMC diagnostic test instrument, the validity results were 95.7% (very valid) and the teacher response questionnaire results were 95% (very high).
