Library database management carried out at UNTIRTA Industrial Engineering is still done manually and is not well systemized. This is not in accordance with UNTIRTA's vision, namely a smart and green campus. Therefore, research was carried out on designing an information system for borrowing and returning books in the library of the Industrial Engineering department. This research was conducted to identify the scope and analysis of the problem, determine the flow of data flow diagrams and find out the test results of the designs that have been made. The research method uses the FAST method (Framework of the Application System Thinking). The FAST method has several stages of problem solving, including defining the scope, problem analysis, system requirements analysis, designing a system model, implementing the model, and testing. Design a system model using use case diagrams, data flow diagrams, and entity relationship diagrams. Model implementation consists of creating a website-based application using the PHP programming language and database management using XAMPP. Testing was carried out using a usability scale system. The average SUS score obtained was 80.1667. These results show that the application can be used well.    Keyword: database, FAST, library, system usability scale
