This study aims to determine the Transformational Leadership of the Head of Women's Service at the Cilegon City Health Office. This research uses mixed methods, which is research that combines quantitative and qualitative data analysis. The type of research design used in this research is Explanatory Sequential Design. In this study using Bass and Avolio's transformational leadership theory (1994) which consists of Idealized Influence (ideal influence/charisma), Inspirational Motivation, Intellectual Stimulation and Individualized Consideration. Data were collected by questionnaire, interview, observation and documentation. The results showed that the head of the Cilegon City Health Office had brought changes and encouraged the health service organization. This is based on the results of research conducted at the Cilegon City Health Office and health centers, it can be seen that the answers of 145 respondents with descriptive percentage data analysis from each statement of each transformational leadership indicator resulted in an average score of 79.88%, this score is in the interval range of 62.50% - 81.24% or is in the “Transformational” criteria. The head of service has fulfilled the four indicators of transformational leadership because the head of service is a leader who is used as an example by his employees, always provides motivation and inspiration, always encourages his employees to be able to think creatively and innovatively, the head of service encourages employees to be able to think of creative solutions in dealing with problems, and pays attention to his employees by providing empowerment.
