The purpose of this research is to further explain how the success  of the Healthy City Program in Realizing Healthy and Independent Communities in  Serang City is based on the concept of program evaluation and using Leo  Agustino's evaluation model theory. This research uses a descriptive method with  a qualitative approach. The results of this study indicate that the implementation of  the Healthy City Program in Realizing Healthy and Independent Communities in  Serang City has not been running optimally, because there are still several  problems, such as: lack of coordination between regional apparatus organizations,  less active Healthy City Forum at the Serang City level, District Communication  Forum and Kelurahan Working Group, lack of budget in program implementation,  low community participation in Healthy City Program activities, and etc.
