description: Fatahillah Kindergarten is currently building a play area. However, there is a lack of awareness of the importance of occupational safety and health in carrying out work. This research was conducted to find out what sources of danger and causes of work accidents occur, and to find out how to control the risk of danger to reduce the occurrence of accidents. This research method is in the form of Hazard Identification and Risk Assessment (HIRA) to identify hazards and assess risk levels. Fault Tree Analysis (FTA) to analyze the root causes of hazards. The results of this research found 22 potential dangers in the construction of play areas with a risk value of 13 in the low risk (L) category, 6 in the medium risk (M) category, 2 in the high risk (H) category and 1 in the extreme risk (E) category. . The root cause of work accidents comes from the 1 highest score, namely the worker falling from the support. The root causes of the problem of falling from supports include workers wearing sandals, workers not wearing safety equipment, unbalanced bodies and slippery supports. Budgeting for work support facilities such as shoes, safety equipment and concentrating when carrying heavy iron frames, carrying castings, strong winds and avoiding working when supports or stairs are damp due to exposure to dew and stairs are exposed to castings, workers must be in fit condition and workers must concentrate on work It is also mandatory to wear complete PPE so that workers are safer and avoid unwanted things so they can control the risk of danger to reduce the occurrence of work accidents in the construction work of the play area at Fatahillah Kindergarten.
