Abstract

The Application of a 30-Degree Head-Up Position to Enhance Oxygen Saturation in Hemorrhagic Stroke Patients in the Intensive Care Unit (ICU) at Banten Provincial General Hospital Background: Hemorrhagic stroke is an acute condition caused by the rupture of cerebral blood vessels, resulting in bleeding within brain tissue or the subarachnoid space. Initial management includes positioning the head at a 30-degree elevation, which helps improve oxygen saturation by facilitating lung expansion and reducing intracranial pressure. This intervention supports cerebral perfusion and prevents neurological damage. Objective: To examine the implementation of a 30degree head up position to improve oxygen saturation in hemorrhagic stroke patients in the ICU of Banten Provincial General Hospital. Methods: This study employed the nursing process approach, including assessment, diagnosis, planning, implementation, evaluation, and documentation. Data were collected through interviews, observations, and medical records. Results: After five days of intervention-two days of assessment and three days of implementation-the first patient's oxygen saturation increased from 90% to 99%, and the second patient's saturation increased from 96% (on a ventilator) to 100%. The first patient's level of consciousness improved from delirium to apathy. Conclusion: The 30-degree head-up position is effective in improving oxygen saturation in patients with hemorrhagic stroke. Recommendation: This position can be routinely applied in the care of hemorrhagic stroke patients.

Penatalaksanaan awal mencakup posisi head up 30 derajat, berperan dalam meningkatkan saturasi oksigen dengan memfasilitasi ekspansi paru dan menurunkan tekanan intrakranial. Intervensi ini mendukung perfusi serebral dan mencegah kerusakan neurologis. Tujuan: Untuk Mengetahui Penerapan Posisi Head Up 30 Derajat Untuk Meningkatkan Saturasi Oksigen Pada Stroke Hemoragik di ICU RSUD Provinsi Banten. Metode: Menggunakan pendekatan proses keperawatan pengkajian, diagnosa, perencanaan, implementasi, evaluasi, dokumentasi. Data diperoleh melalui wawancara, observasi, dan rekam medis. Hasil: Setelah 5 hari intervensi, 2 hari pengkajian, 3 hari implementasi, saturasi oksigen pasien pertama meningkat dari 90% menjadi 99%, pasien kedua dari 96% (dengan ventilator) menjadi 100%. Kesadaran pasien pertama meningkat dari delirium menjadi apatis. Kesimpulan: Posisi head up 30° efektif meningkatkan saturasi oksigen pada stroke hemoragik. Saran: Posisi ini dapat digunakan rutin dalam perawatan stroke hemoragik.
