Abstract
Background: Chronic Kidney Disease (CKD) often leads to fluid overload (hypervolemia) due to impaired excretion, which increases hydrostatic pressure in the pulmonary capillaries. This condition causes pulmonary edema, resulting in shortness of breath and an ineffective breathing pattern in patients. Objective: To evaluate the effectiveness of applying the 45-degree semi-Fowler’s position in reducing dyspnea among CKD patients experiencing ineffective breathing patterns. Case Study: This case study involved two patients treated in Garuda Ward, 4th Floor, RSUD Banten Province. Both were provided the semi-Fowler’s position (45° elevation) as an intervention, based on Evidence Based Nursing (EBN) recommendations. Results: The first patient showed clinical improvement, including decreased respiratory rate, increased oxygen saturation (SpO₂), reduced nasal flaring and use of accessory muscles, and reported feeling more comfortable. The second patient also reported comfort while in the semi-Fowler’s position; however, the intervention was less effective overall due to the patient’s refusal to undergo hemodialysis. Conclusion: The semi-Fowler’s position can effectively reduce dyspnea and enhance comfort in CKD patients with ineffective breathing patterns. However, optimal outcomes require additional supportive interventions, such as fluid intake monitoring and hemodialysis therapy.

| Additional Information: | Latar Belakang: Gagal ginjal kronik dapat menyebabkan hipervolemia atau penumpukan cairan pada tubuh. Hipervolemia pada CKD menyebabkan tekanan hidrostatik dalam pembuluh darah paru menjadi meningkat sehingga cairan keluar ke jaringan paru dan terjadilah edema paru yang akan menimbulkan sesak napas. Tujuan: Untuk mengetahui efektifitas dari penerapan posisi semi fowler 45o untuk mengurangi sesak napas pada pasien CKD. Metode: Dalam penelitian ini menggunakan metode studi kasus yang dilakukan kepada 2 pasien dan dilakukan tindakan memberikan posisi semi fowler 45o sesuai dengan jurnal EBN. Hasil: Pada pasien pertama terbukti efektif karena frekuensi napas membaik, SPO2 meningkat, cuping hidung dan penggunaan otot bantu napas berkurang, pasien juga mengatakan nyaman, namun pada pasien kedua posisi semi fowler kurang efektif dikarenakan pasien juga menolak untuk dilakukan terapi hemodialisis, tetapi pasien mnegatakan nyaman dengan pemberian posisi semi fowler Kesimpulan: Pengaturan posisi semi fowler pada pasien CKD dengan masalah pola napas tidak efektif mampu untuk mengurangi sesak napas dan memberikan rasa nyaman, namun harus didukung juga dengan tindakan lainnya seperti pemantauan asupan cairan dan terapi hemodialisis. | ||||||
