Abstract
Congestive heart failure (CHF) is a condition in which the heart is unable to pump blood effectively, causing decreased cardiac output and symptoms such as shortness of breath. Objective: to evaluate the effectiveness of the semi-fowler position in increasing oxygen saturation in CHF patients with decreased cardiac output in the Emergency Department of Banten Provincial Hospital. The main intervention was the application of the semi-fowler position (head elevation 30–45°) for three days. Data were collected through interviews, observations, and medical records. Results: showed an increase in oxygen saturation from 93% to 99% in the first patient and from 94% to 98% in the second patient, accompanied by a decrease in complaints of shortness of breath. Evaluation on the third day showed an improvement in cardiac output. These findings are in line with previous studies showing that the semi-fowler position can improve oxygenation and reduce cardiac workload in CHF patients. Conclusion: The application of the semi-fowler position is effective in increasing oxygen saturation and reducing shortness of breath in CHF patients with decreased cardiac output. Suggestion: The semi-fowler position is recommended as a non-pharmacological nursing intervention in the management of CHF patients in the emergency department.

Tujuan: untuk mengevaluasi efektivitas posisi semi-fowler dalam meningkatkan saturasi oksigen pada pasien CHF dengan penurunan curah jantung di Instalasi Gawat Darurat RSUD Provinsi Banten. Intervensi utama adalah penerapan posisi semi-fowler (elevasi kepala 30–45°) selama tiga hari. Data dikumpulkan melalui wawancara, observasi, dan rekam medis. Hasil: menunjukkan peningkatan saturasi oksigen dari 93% menjadi 99% pada pasien pertama dan dari 94% menjadi 98% pada pasien kedua, disertai penurunan keluhan sesak napas. Evaluasi pada hari ketiga menunjukkan perbaikan curah jantung. Temuan ini sejalan dengan penelitian sebelumnya yang menunjukkan bahwa posisi semi-fowler dapat meningkatkan oksigenasi dan mengurangi beban kerja jantung pada pasien CHF. Kesimpulan: Penerapan posisi semi-fowler efektif dalam meningkatkan saturasi oksigen dan mengurangi sesak napas pada pasien CHF dengan penurunan curah jantung. Saran: Posisi semi-fowler direkomendasikan sebagai intervensi keperawatan non-farmakologis dalam penanganan pasien CHF di unit gawat darurat. | ||||||
