Abstract

Background : Tuberculosis of bones and joints is a rare type of tuberculosis. Complications and serious health impacts can occur due to infection of this disease. This study aims to determine the relationship between age, gender, HIV, BMI and comorbid factors with the incidence of bone and joint tuberculosis at Dr Drajat Prawiranegara Hospital, Serang. Methods : This research is an observational study with analytical methods and a cross-sectional approach using medical record data of tuberculosis patients at Dr. Drajat Prawiranegara Regional Hospital, Serang in the period of January 2019-December 2023. The number of samples obtained was 110 tuberculosis patients, using total sampling techniques and using univariate and bivariate analysis via Chi Square and Fisher tests. Results : Patients were predominantly of productive age (82.7%) with an average age of 47 years, 64.5% of male patients, most nutritional status was in the normal category (48, 2%), and 58.2% of patients had no comorbidities. There was a non-significant relationship with the variables age and gender, and status and a significant relationship with underweight BMI (p=0.002 OR 0,13) and comorbidities (p<0.001, OR 0.14). Conclusion : Underweight BMI reduces the risk of developing bone and joint TB, and people who have comorbid metabolic diseases are less at risk of developing bone and joint TB because there are other influencing factors, namely vitamin D deficiency.

Additional Information: Pendahuluan : Tuberkulosis tulang dan sendi merupakan jenis yang langka dari penyakit tuberkulosis. Komplikasi dan dampak kesehatan yang berat dapat terjadi akibat infeksi penyakit ini. Penelitian ini bertujuan untuk mengetahui hubungan faktor usia, jenis kelamin, IMT, dan komorbid dengan kejadian tuberkulosis tulang dan sendi di RSUD dr Drajat Prawiranegara Serang. Metode : Penelitian ini merupakan penelitian observasional dengan metode analitik dan pendekatan potong lintang/cross sectional menggunakan data rekam medis pasien tuberkulosis di RSUD dr Drajat Prawiranegara Serang pada rentang waktu Januari 2019-Desember 2023. Jumlah sampel yang didapatkan sebanyak 110 orang tuberkulosis dengan teknik total sampling dan menggunakan analisis univariat dan bivariat melalui uji Chi Square dan Fisher. Hasil : Pasien didominasi usia produktif (82,7%) dengan rata-rata usia 47 tahun, pasien laki-laki sebanyak 64,5%, status gizi terbanyak berada pada kategori normal (48,2%), dan 58,2% pasien tidak memiliki komorbid. Terdapat hubungan yang tidak signifikan pada variabel usia dan jenis kelamin, dan hubungan yang signifikan pada IMT (p=0,002 OR 0,13) dan komorbid (p<0,001, OR 0,14). Kesimpulan : IMT underweight menurunkan risiko kejadian TB tulang dan sendi, dan orang yang memiliki komorbid penyakit metabolik lebih tidak beresiko mengalami TB tulang dan sendi karena terdapat faktor lain yang berpengaruh yaitu defisiensi vitamin D.
