Abstract
This research identifies and analyzes risks at PT. Ashoka Waja Wisesa, revealing 32 risk events and 18 risk agents influencing the company. Utilizing the fuzzy-HOR method, 9 priority risk sources were identified, categorized into A (0-50%) and B (50-80%), such as technical errors by employees and limited production tools. In the HOR phase 2 processing, 18 preventive actions were proposed, ranked from highest to lowest ETDk, including technical job performance evaluation, product double checking, and SOP implementation training. The research conclusion provides profound insights into the risks faced by the company and offers relevant preventive solutions.

Menggunakan metode fuzzy-HOR, didapatkan 9 sumber risiko prioritas, terbagi dalam kategori A (0-50%) dan B (50-80%), seperti kesalahan teknis oleh karyawan dan terbatasnya alat produksi. Pada tahap HOR fase 2, diperoleh 18 tindakan pencegahan dengan ETDk tertinggi hingga terendah, termasuk evaluasi kinerja teknis, double checking produk, dan pelatihan implementasi SOP. Kesimpulan penelitian memberikan wawasan mendalam terhadap risiko yang dihadapi perusahaan dan menawarkan solusi pencegahan yang relevan. | |||||||||
