Abstract
Nursing care is a process carried out by a nurse in providing health services to individuals, families and communities with the aim of promoting, maintaining and restoring health. Nursing care involves assessment, diagnosis, planning, implementation, and evaluation to ensure that patients receive the best and most effective care. Diarrhea is a disease characterized by changes in the shape and consistency of stool, as well as an increase in the frequency of defecation from usual, up to 3 or more times a day. WHO (2017) there are around 1.7 billion cases of diarrhea with a death rate of around 525,000 children every year. Overall, the number of diarrhea cases in Banten province in 2020 was 18.4% (Banten Province Health Office 2021). Based on a preliminary study conducted at RSUD DR. Drajat Prawairanegara Serang Banten, there were 285 cases of diarrhea in children from August 2023-January 2024. Play therapy is an intervention that can be applied to suppress anxiety and uncooperative attitudes in children. The purpose of this case study is to describe nursing care. Anxiety in Preschool Age Diarrhea Children with Play Therapy Actions, using a nursing care format which includes assessment, diagnosis, intervention, implementation and evaluation. This research is a descriptive method using a case study with a research sample of 2 child clients who experienced diarrhea. This research was conducted using data collection techniques in the form of interviews and observations. During the assessment, data was found on client 1, namely loose stools with a frequency of <5 times a day, the client was fussy, restless, weak, had difficulty sleeping, pale face. In client 2, data was found to defecate ±7 times a day, restless, fussy, cried easily, had difficulty concentrating, looked tense, face pale. The nursing diagnosis for client 1 and client 2 is anxiety related to a situational crisis. The nursing intervention is reducing anxiety with play therapy. Nursing implementation is the same as the intervention that has been prepared. Evaluation of clients 1 and 2, nursing problems resolved on day 4.

| Additional Information: | Asuhan keperawatan merupakan suatu proses yang dilakukan oleh seorang perawat dalam memberikan pelayanan kesehatan kepada individu, keluarga, masyarakat dengan tujuan mempromosikan, memelihara dan mengembalikan kesehatan. Asuhan keperawatan melibatkan pengkajian, diagnosa, perencanaan, implementasi, dan evaluasi untuk memastikan bahwa pasien mendapatkan perawatan yang terbaik dan efektif. Diare merupakan penyakit yang ditandai dengan perubahan bentuk dan konsistensi tinja, serta bertambahnya frekuensi buang air besar dari biasanya hingga 3 kali atau lebih dalam sehari. WHO (2017) terdapat sekitar 1,7 milyar kasus diare dengan angka kematian sekitar 525.000 pada anak setiap tahunnya. Secara keseluruhan jumlah kasus diare di provinsi banten pada tahun 2020 yaitu sebanyak 18,4% (Dinkes Provinsi Banten 2021). Berdasarkan studi pendahuluan yang dilakukan di RSUD DR. Drajat Prawairanegara Serang Banten, terdapat 285 kasus diare pada anak dari bulan Agustus 2023-Januari 2024. Terapi bermain sebagai salah satu intervensi yang bisa diterapkan untuk menekan ansietas dan sikap tidak kooperatif pada anak. Tujuan dari studi kasus ini adalah menggambarkan Asuhan Keperawatan. Ansietas Pada Anak Diare Usia Prasekolah dengan Tindakan Terapi Bermain, menggunakan format asuhan keperawatan yang meliputi pengkajian, diagnosa, intervensi, implementasi, dan evaluasi. Penelitian ini adalah metode deskriptif menggunakan studi kasus dengan sampel penelitian pada 2 klien anak yang mengalami diare. Penelitian ini dilakukan dengan menggunakan teknik pengumpulan data berupa wawancara dan observasi. Saat pengkajian ditemukan data pada klien 1 yaitu BAB cair dengan frekuensi <5x dalam sehari, klien rewel, gelisah, lemas, sulit tidur, wajah pucat. Pada klien 2 ditemukan data BAB ±7X dalam sehari, gelisah, rewel, mudah menangis sulit berkonsentrasi, tampak tegang, wajah pucat. Diagnosa keperawatan pada klien 1 dan klien 2 yaitu ansietas berhubungan krisis situasional. Intervensi keperawatan yaitu reduksi ansietas dengan tindakan terapi bermain . Implementasi keperawatan sama dengan intervensi yang telah disusun. Evaluasi pada klien 1 dan 2 masalah keperawatan teratasi pada hari ke 4. | |||||||||
