Abstract
Introduction Premature rupture of membranes (PROM) is a labor disorder in the form of rupture of the amniotic membrane before labor occurs. According to the World Health Organization (PROM) 2014, the incidence of PROM in the world is 5-10% of total deliveries, while the proportion of PROM in Indonesia based on Riskesdas 2018 is 5.6% and is the most common delivery disorder. Banten ranks second with a proportion of 7%, which exceeds the national average. PROM can affect poor fetal outcomes which can be measured by the APGAR score. In this study, the researchers wanted to know the relationship between the duration of PROM and the APGAR Score of Infants in PROM Patients at Banten Hospital in 2023. Method This study used a cross-sectional design through a retrospective approach to determine the relationship between the duration of PROM and the 5th minute APGAR score. The inclusion criteria in this study were pregnant women at Banten Regional Hospital for the period January 1-December 31, 2023 who were diagnosed with PROM as evidenced by anamnesis, physical examination, and support contained in medical records. Exclusion criteria included congenital malformations, delivery of twins, fetal death during labor, and intrauterine growth restriction (IUGR). Sampling was done through proportional random sampling approach. Data processing was performed using the chi-square or fisher statistical test using the p-value limit of meaningfulness. Result There were 106 samples obtained in this study. The proportion of PROM duration >12 hours was 53.8% and APGAR score <7 was 17%. Based on the results of statistical tests using chi-square, there is a significant relationship between the duration of PROM > 12 hours with the 5th minute APGAR score in mothers with PROM at Banten Regional Hospital in 2023 (p=0.025) with PR (IK95%) 3.00 (1.06-8,54)]). In addition, it is known that there is a significant relationship between maternal nutritional status measured using upper arm circumference (MUAC) and birth weight to the 5th minute APGAR score with a value of p=0.03 and p=0.07. Conclusion The duration of PROM has a significant relationship with the 5th minute APGAR score in laboring patients with PROM at Banten Hospital in 2023.

| Additional Information: | Latar Belakang Ketuban pecah dini (KPD) adalah gangguan persalinan berupa pecahnya selaput ketuban sebelum terjadinya persalinan. Menurut World Health Organization (WHO) 2014 insiden KPD di dunia sebesar 5-10% dari total persalinan, sedangkan proporsi KPD di Indonesia berdasarkan Riskesdas 2018 sebesar 5,6% dan menjadi gangguan persalinan terbanyak. Banten menempati urutan kedua dengan proporsi KPD 7% yang melebihi rata-rata KPD nasional. KPD dapat memengaruhi luaran janin buruk yang dapat diukur dengan skor APGAR. Dalam penelitian ini peneliti ingin mengetahui hubungan lama KPD dengan Skor APGAR Bayi pada Pasien KPD Di RSUD Banten Tahun 2023. Metode Penelitian ini menggunakan desain cross-sectional melalui pendekatan retrospektif untuk mengetahui hubungan antara lama KPD dengan skor APGAR menit ke-5. Kriteria inklusi dalam penelitian ini, yaitu ibu bersalin di RSUD Banten periode 1 Januari-31 Desember 2023 yang terdiagnosis KPD dibuktikan dengan anamnesis, pemeriksaan fisik, dan penunjang yang terdapat di dalam rekam medis. Kriteria eksklusi meliputi, adanya malformasi kongenital, melahirkan anak kembar, janin mati saat persalinan, dan intrauterine growth restriction (IUGR). Pengambilan sampel dilakukan melalui pendekatan proportional random sampling. Pengolahan data dilakukan menggunakan uji statistik chi-square atau fisher dengan menggunakan batas kebermaknaan nilai p. Hasil Sampel yang didapatkan dalam penelitian ini sebanyak 106 sampel. Proporsi lama KPD >12 jam sebesar 53,8% dan skor APGAR <7 sebesar 17%. Berdasarkan hasil uji statistik menggunakan chi-square terdapat hubungan bermakna antara lama KPD >12 jam dengan skor APGAR menit ke-5 pada ibu dengan KPD di RSUD Banten tahun 2023 (p=0,025) dengan PR (IK 95%) 3,00 (1,06-8,54)]). Selain itu, diketahui terdapat hubungan bermakna antara status gizi ibu yang diukur menggunakan lingkar lengan atas (LILA) dan berat badan lahir terhadap skor APGAR menit ke-5 dengan nilai p=0,03 dan p=0,07. Kesimpulan Lama KPD memiliki hubungan bermakna terhadap skor APGAR menit ke-5 pada pasien bersalin dengan KPD di RSUD Banten tahun 2023. | |||||||||
