Abstract
Synthetic resin product is the raw material for the needs of industries such as textiles, paint, ink, and automotive, one of the industries that produce it is PT XYZ. The involvement of the company as a pioneer in the production of synthetic resins for domestic needs, and most of Asia require the company to produce quality products and maintaining a smooth supply chain. To maintain a smooth and produce quality products, so companies need to pay attention and identify risks that could potentially arise on the supply chain using the SCOR model for mapping activities. The identification of risks is carried out to find out and give an assessment of severity, occurrence, and correlation against risk event and risk agent happened to the concept of Failure Mode Effect Analysis (FMEA) and House Of Quality (HOQ), the use of this method is known as the House Of Risk. Based on the results of the identification of risks in the supply chain of synthetic resin PT XYZ there are 29 risk event and 28 risk agent. The results of the calculation of aggregate risk potential at House of risk 1 elected 15 risk agent to do the action on mitigation. Then retrieved 12 mitigation actions designed to reduce the risk of agent, the selection is done by considering the resources, policy management, and cost the company owned.

| Additional Information: | Produk resin sintetis merupakan bahan baku untuk kebutuhan industri seperti industri tekstil, cat, tinta, dan otomotif, salah satu industri yang memproduksinya yaitu PT XYZ. Keterlibatan perusahaan sebagai pelopor produksi resin sintetis untuk kebutuhan domestik, dan sebagian Asia mengharuskan perusahaan untuk menghasilkan produk berkualitas dan menjaga kelancaran rantai pasoknya. Untuk menjaga kelancaran dan menghasilkan produk berkualitas, maka perusahaan perlu memperhatikan dan mengidentifikasi risiko yang berpotensi muncul pada rantai pasok menggunakan model SCOR untuk pemetaan aktivitasnya. Identifikasi risiko dilakukan untuk mengetahui dan memberi penilaian severity, occurrence, dan correlation terhadap risk event dan risk agent yang terjadi dengan konsep Failure Mode Effect Analysis (FMEA) dan House Of Quality (HOQ), penggunaan metode ini dikenal sebagai House Of Risk. Berdasarkan hasil identifikasi risiko pada rantai pasok resin sintetis PT XYZ terdapat 29 risk event dan 28 risk agent. Selanjutnya dari hasil perhitungan aggregate risk potential pada House of risk 1 terpilih 15 risk agent untuk dilakukan aksi mitigasi. Kemudian diperoleh 12 aksi mitigasi yang dirancang untuk mengurangi terjadinya agen risiko, pemilihan ini dilakukan dengan mempertimbangkan sumber daya, kebijakan manajemen, dan kebutuhan biaya yang dimiliki perusahaan. | |||||||||
