Abstract
PT. X is a manufacturing company that produces medium paper. Medium paper is widely used as packaging for other products, for example mineral water packaging boxes, packaged instant noodles and others. In this supply chain activity, there are opportunities for risks to arise which will disrupt the supply chain. For this reason, company management needs to carry out a management action which can reduce or prevent risks from occurring. This study aims to identify any risks that may arise or have occurred in the supply chain of PT. X and knowing which risks will be prioritized in risk mitigation so that they can determine the most appropriate mitigation actions to prevent or reduce the occurrence of these risks. The method used in this research is the fuzzy house of risk method. Based on the results obtained, 24 risk events and 28 risk agents were identified and there were 18 proactive actions that were proposed to the company to be able to handle risk agents which are a priority for the mitigation process in this study.

| Additional Information: | PT. X merupakan salah satu perusahaan manufaktur yang memproduksi kertas medium. Kertas medium banyak digunakan sebagai packaging dari produk lainnya, misalkan box kemasan air mineral, mie instan kemasan dan lain-lain.Pada kegiatan rantai pasok ini terdapat peluang munculnya risiko yang akan mengganggu jalannya rantai pasok. Untuk itu manajemen perusahaan perlu melakukan suatu aksi manajemen dimana dapat mengurangi atau mencegah timbulnya risiko. Penelitian ini bertujuan untuk mengidentifiksi risiko apa saja yang berkemungkinan muncul ataupun yang pernah terjadi pada rantai pasok PT. X serta mengetahaui risiko manakah yang akan menjadi prioritas dalam mitigasi risiko sehingga dapat menentukan aksi mitigasi yang paling tepat untuk mencegah ataupun mengurangi timbulnya risiko tersebut. Metode yang digunakan dalam penelitian ini adalah metode fuzzy house of risk. Berdasarkan hasil yang diperoleh teridentifikasi 24 risk event serta 28 risk agent dan terdapat sebanyak 18 proactive acton yang diusulkan kepadapihak perusahaan untuk dapat menangani risk agent yang menjadi prioritas untuk proses mitigasi di dalam penelitian ini. | |||||||||
