Abstract

The production process in the steel industry is inseparable from the risk of unsafe acts and unsafe conditions. PT XYZ has 5 areas of steel HRC production process. One of these areas is the down coiler area where there is still manual work taking place, such as binding, writing labels and cutting samples on HRC steel. This work carries the risk of work related accidents. Therefore it was found necessary to identify these risks in detail, using the HIRARC and FTA methods. Based on recent research that has been carried out, there are 58 potential hazards, 14 potential hazards with low risk and 44 potential hazards with medium risk. As well as proposed risk management in the form of substitution (replacing dies), planning ( adding blowers ) the administration side of the process (revising the oil cleaning schedule) and PPE (mask, safety helmet, and safety shoes). There are 4 factors that can cause danger of iron filings, such as machines that are in operation, late schedule of cleaning the area, employees that are not wearing the correct PPE when in the surrounding area of operating machinery, and employees who are not carrying out provided safety instructions. There are 4 suggestions for improvements that can be made, such as that LED display monitor must always be in good condition and operating, carrying out routine maintenance, creating an accurate visual check schedule and installing "Use Mask" signs in the finishing mill of production area to remind employees of mandatory safety measures and wearing the correct PPE. As well as informing employees of safety regulations and instructions on a regular basis.

Additional Information: Proses produksi di industri baja tidak terlepas dari adanya risiko terhadap tindakan tidak aman dan kondisi tidak aman. PT XYZ memiliki 5 area proses produksi baja HRC, salah satunya area down coiler yang masih terdapat pekerjaan manual yaitu pengikatan, penulisan label, dan pemotongan sample pada baja HRC. Pekerjaan tersebut memiliki risiko kecelakaan kerja bagi para pekerjanya, oleh karena itu dilakukan pengidentifikasian lebih rinci menggunakan metode HIRARC dan FTA. Berdasarkan penelitian yang telah dilakukan terdapat 58 potensi bahaya, dengan tingkat risiko rendah sebanyak 14 potensi bahaya dan tingkat risiko sedang sebanyak 44 potensi bahaya, serta usulan penanganan risiko berupa substitusi (penggantian dies), perancangan (penambahan blower), administrasi (merevisi jadwal cleaning oli) dan APD (masker, helm safety, dan sepatu safety). Terdapat 4 faktor penyebab dari bahaya serbuk besi yaitu aktivitas mesin yang sedang beroperasi, telatnya pembersihan area, pekerja tidak menggunakan masker saat berada diarea sekitar, dan pekerja tidak melakukan safety induction. Terdapat 4 usulan perbaikan yang dapat dilakukan yaitu LED display monitor harus selalu dalam keadaan nyala dan rutin melakukan maintenance, membuat jadwal visual check secara tepat, pemasangan rambu “Gunakan Masker” pada area finishing mill agar pekerja merasa adanya peringatan secara visual, dan penyampaian informasi tentang safety induction secara tepat dan rutin.
