Abstract

Diarrhea is a common occurrence in the community and is more prone to attack toddlers. Diarrhea is a symptom with the process of defecating (defecation) with liquid stools and a frequency of more than 3 times a day accompanied by blood or not, causing a lack of fluids and electrolytes. In 2017 the child mortality rate due to diarrhea was around 525,000 cases. The incidence of diarrhea in the flamboyant room of dr. Dradjat Prawiranegara Serang from July to December 2021 as many as 143 cases. Hypovolemia can occur due to fluid loss through the skin, kidneys, gastrointestinal tract, bleeding so that it can cause hypovolemic shock (Tarwoto & Wartonah, 2015). Fluid therapy is a therapy that greatly affects the success of critically ill patients. In general, fluid and electrolyte therapy can be enteral or parenteral. Appropriate handling in regulating fluid balance in diarrheal clients, one of which is monitoring IWL (insensible water loss), intake and output of patients every day, to measure and monitor fluid intake and output every day. While other treatment by providing oral fluid intake. The purpose of this paper is to describe nursing care for hypovolaemic children with diarrhea by administering fluid therapy in the flamboyant room of dr. Dradjat Prawiranegara Serang. The research method is a case study with a descriptive type of research using a nursing care approach which includes assessment, nursing diagnoses, planning, implementation, and nursing evaluation. Participant 1 is 5 years old and participant 2 is 1 year old. The results of the study showed that participants had different symptoms, namely in participant 1, 5x liquid bowel movements with dregs, 3x vomiting, while in participant 2, 3x liquid bowel movements with dregs and nausea. The nursing diagnosis established for both participants was hypovolemia related to active fluid loss. The nursing plan is management of hypovolemia. The implementation is carried out according to the intervention, namely checking for signs and symptoms of hypovolemia, monitoring fluid intake and output, calculating fluid requirements, providing oral fluid intake, recommending increasing oral fluid intake, collaborating with isotonic IV fluid therapy. The evaluation of participant 1 was resolved on day 2, while participant 2 was resolved on day 3. The conclusion of giving fluid therapy to clients with diarrhea is very important, so it is hoped that nurses will further improve services in providing nursing care and pay attention to monitoring fluid balance in clients.
