Abstract
Diabetes Mellitus is a metabolic disease characterized by an increase in blood glucose levels caused by disturbances in insulin production, insulin sensitivity, or both. Glucose in large quantities causes the blood to become concentrated and blood flow becomes not smooth due to constriction of blood vessels. Blood flow that is not smooth causes neuropathy in peripheral nerves because the supply of oxygen and nutrients to the tissue is blocked, causing diabetic ulcers and wounds that are difficult to heal. In the last decade of research, it was found that honey is a wound dressing that has antibacterial, anti-inflammatory, antioxidant and autolytic debridement effects which are effectively used in accelerating the wound healing process so as to reduce amputation rates in diabetic ulcer patients. The purpose of the study was to carry out nursing care for skin integrity disorders in patients with type 2 diabetes mellitus and to find out how the effectiveness of honey therapy wound care on the healing process of diabetic ulcers. The research method uses a case study method with 2 clients as respondents which was carried out from January to June 2022 at the cendrawasih 2 room Banten Provincial Hospital. Nursing care was carried out for three days with one of the interventions, namely honey therapy wound care. The intervention is carried out once a day and takes 30-60 minutes. This intervention proved to be effective with the result of increasing skin integrity with the criteria of increasing peripheral perfusion, decreasing pain, decreasing redness, decreasing necrosis and decreasing infection. In conclusion, the results of nursing care carried out by researchers are in accordance with previous studies that honey is effective in accelerating the healing of diabetic ulcers.

| Additional Information: | Diabetes Melitus merupakan suatu penyakit metabolik yang ditandai dengan adanya peningkatan kadar glukosa dalam darah yang disebabkan oleh gangguan pada produksi insulin, sensitifitas insulin, maupun keduanya. Glukosa dengan jumlah banyak menyebabkan darah menjadi pekat dan aliran darah menjadi tidak lancar karena terjadi penyempitan pembuluh darah. Aliran darah yang tidak lancar menyebabkan neuropati pada saraf perifer karena suplai oksigen dan nutrisi kejaringan terhambat sehingga menimbulkan ulkus diabetik dan luka sukar sembuh. Pada penelitian dekade terakhir diketahui bahwa madu merupakan wound dressing yang memiliki efek antibakterial, antiinflamasi, antioksidan dan debridemen autolitik yang efektif digunakan dalam mempercepat psoses penyembuhan luka sehingga dapat mengurangi tingkat amputasi pada pasien ulkus diabetik. Tujuan penelitian melaksanakan asuhan keperawatan gangguan integritas kulit pada pasien diabetes melitus tipe 2 dan untuk mengetahui bagaimana efektivitas perawatan luka terapi madu terhadap proses penyembuhan luka ulkus diabetik. Metode penelitian menggunakan metode studi kasus dengan 2 pasien sebagai responden yang dilaksanakan pada bulan Januari sampai dengan Juni 2022 di ruang cendrawasih RSUD Provinsi Banten. Asuhan keperawatan dilakukan selama tiga hari dengan salah satu intervensi yaitu melakukan perawatan luka terapi madu. Intervensi dilakukan setiap satu kali dalam sehari dengan membutuhkan waktu 30-60 menit. Intervensi ini terbukti efektif dengan hasil integritas kulit meningkat dengan kriteria hasil perfusi perifer meningkat, nyeri menurun, kemerahan menurun, nekrosis menurun dan infeksi menurun. Kesimpulannya hasil asuhan keperawatan yang dilakukan peneliti sejalan dengan penelitian sebelumnya bahwa madu efektif dalam mempercepat proses penyembuhan luka ulkus diabetik. | |||||||||
