Abstract
Workplace accidents is an adverse event, not only affect the victim but also the impact on the company, cause the cessation of the production process and can lead to a bad impression on consumers. Construction services will have a huge risk of occupational accidents, because the work was related to heavy equipment. In addition, factor workers who perform unsafe behavior (unsafe behavior) also becomes one of the factors that could cause accidents. Unsafe behavior is behavior that leads to accidents. Based on data from occupational accidents in 2014 PT. Cilegon Fabricators, there are 36 times the accidents that occur on the carrier for the year. Accidents most often happen to be in the shop working environment 5 & 6 and shop 1. One way to reduce workplace accidents due to unsafe behavior is to identify the root causes of accidents due to unsafe behavior based safety and health issues then recommends improvements. Problem-solving method in this research begins with a reconstruction because the incidence of the problem using Root Cause Analyze (RCA), Determining the Risk Priority Number (RPN) using Failure Mode and Effect Analysis (FMEA) perform repair recommendations based on priorities using 5W + 1H. 3 cause of the problem customs workers perform unsafe acts (unsafe behavior), do not follow the workflow and lack of socialization with the value of RPN 60, placing material on irregular and warehouse overcapacity with a value of RPN 50, lack of supervision and safety officer using shift schedule with a value of RPN 45. The proposal is to install workflow improvement per work area, providing operator training on distribution schedule K3 safety officer

| Additional Information: | Kecelakaan kerja adalah suatu kejadian yang merugikan, bukan hanya berdampak kepada korban saja namun juga berdampak kepada perusahaan, menyebabkan terhentinya proses produksi dan dapat menimbulkan kesan buruk terhadap konsumen. Bidang jasa konstruksi memiliki resiko yang besar akan kecelakaan kerja, karena pekerjaan yang dilakukan berhubungan dengan alat berat. Disamping itu, faktor manusia yang melakukan perilaku tidak aman (unsafe behaviour) juga menjadi salah satu faktor yang dapat menyebabkan terjadinya kecelakaan kerja. Unsafe behavior adalah perilaku yang mengarah pada kecelakaan. Berdasarkan data kecelakaan kerja pada tahun 2014 PT. Cilegon Fabricators, terdapat 36 kali kecelakaan kerja yang terjadi pada operator pada tahun tersebut. Kecelakaan yang paling sering terjadi berada di lingkungan kerja shop 5&6 dan shop 1. Salah satu cara untuk mengurangi kecelakaan kerja akibat unsafe behaviour adalah mengidentifikasi akar penyebab kecelakaan kerja akibat unsafe behaviour berdasarkan permasalahan keselamatan dan kesehatan kerja kemudian merekomendasi perbaikan. Metode penyelesaian masalah pada penelitian ini diawali dengan melakukan rekonstruksi sebab kejadian masalah menggunakan metode Root Cause Analyze (RCA), Menentukan Risk Priority Number (RPN) menggunakan metode Failure Mode and Effect Analysis (FMEA) melakukan rekomendasi perbaikan berdasarkan skala prioritas menggunakan 5W+1H. Tiga penyebab masalah kebiasaan pekerja melakukan tindakan tidak aman (unsafe behaviour), tidak mengikuti workflow dan kurang sosialisasi dengan nilai RPN 60, penempatan material tidak beraturan dan warehouse overcapacity dengan nilai RPN 50, kurangnya pengawasan dan safety officer menggunakan jadwal shift dengan nilai RPN 45.Usulan perbaikannya adalah memasang workflow per area kerja, memberikan pelatihan K3 operator melakukan pembagian jadwal safety officer. | |||||||||
